Subject to applicable law and the practice's final policies, protected health information may be used or disclosed for treatment, payment, and health care operations. Other uses and disclosures may require written authorization.
Our responsibilities
We are required to maintain the privacy and security of protected health information, provide a notice of privacy practices, follow the notice currently in effect, and provide breach notifications as required by law.
